Applications - Address Community Connect (CC) Program is now part of the Reintegration Care Models (RCM) funded by the Toronto Central Local Health Integration Network (TC LHIN). CC Program continues to provide short term, transitional housing coupled with enhanced support services to eligible inpatients being discharged from TC LHIN hospitals.
